The Expert Patient Programme helps people with a long term condition to maintain their health and improve their quality of life.

It is a free course that helps you to take control of your condition.

How To Access the service
The course is self- referring.
It can also can be accessed via your GP, Health Centres, Social Services, other Health Care Professionals, key workers, voluntary groups.

You can print off the above Expert Patient Programme leaflet and post it to the address stated or contact us to be added to future mailings about courses.

Referrals can also be made via your health care provider within Wolverhampton.

The next set of EPP courses start in February/March 2013.
